Some careers leave no room for a quiet struggle. Pilots, mariners, longshoremen, and other safety-sensitive professionals carry the added weight of knowing that asking for help could feel like risking their livelihood. Coastwise provides confidential, expert outpatient therapy built for exactly these situations, so getting well and protecting your career are not at odds.

Every clinician at Coastwise is fully licensed, vetted, and supported by a clinical team. No trainees practicing solo, no guesswork. Meet our team.

Aviation professionals

Pilots working through substance use carry a particular kind of pressure, because the career and the recovery are tied together. We provide the therapy side of that work: confidential individual sessions, relapse prevention, and treatment for whatever sits underneath the using. We do not provide testing, monitoring, or medical evaluations, so if you are in the FAA HIMS process you will need those elsewhere, and we will say so plainly rather than let you assume otherwise.

Maritime and port workers

This is our home, too. Coastwise is in San Pedro, in the harbor, and we understand port work: the shift schedules, the physical toll, the culture, and the very real fear of word getting around. We have worked with longshoremen, ILWU members, and dock workers, and treatment is built around the realities of the waterfront. You do not have to tell anyone at work to get help here.

Getting well and protecting your career are not at odds. Nothing is shared without your written consent, and you decide what is reported and to whom.

How confidential care works here

When your career depends on discretion, the handling of your information matters as much as the therapy itself. Here is how it fits together.

A confidential first call

Reach out for a free, confidential consultation. Nothing leaves the room without your written consent.

Build the right plan

We map out individual therapy and relapse prevention around your schedule, and we are clear from the start about what we do and do not provide.

Coordinate where needed

With your written consent we can coordinate with your physician or treatment team. For substance use, we treat it as outpatient and dual-diagnosis care.

Nothing leaves without your consent

You decide what is shared and with whom, in writing, every time. No employer, union, or licensing body hears from us otherwise.

Stay consistent anywhere

See us in San Pedro or securely online across California, so irregular schedules and travel do not derail your care.

If you are an employer, union, or provider referring a professional, see For Professionals.

Frequently Asked Questions

We provide the confidential outpatient therapy side of that work: individual therapy, relapse prevention, and treatment for the depression, anxiety, or trauma underneath it, with clinicians certified in addiction counseling. We do not provide the testing, monitoring, or medical evaluations the HIMS pathway requires, so pilots in that process will need those through their HIMS AME and monitoring provider. Call (424) 536-3002 and we will be straight with you about what we can and cannot cover.

Confidentiality is the foundation of how we work. Nothing is shared without your written consent, and you decide what is reported and to whom. For port and maritime workers especially, you do not have to tell anyone at work to get help here.

Yes. We are based in San Pedro, in the harbor, and we have worked with longshoremen, ILWU members, dock workers, and others whose schedules, physical demands, and culture are unlike a desk job. Treatment is built around shift work and the realities of the waterfront.
